AccessGet a readable copy of all the data we hold about you.
RectificationCorrect incorrect, incomplete or outdated data.
ErasureDelete the data associated with your email (irreversible).
PortabilityReceive your data in a structured format to bring to another provider.
How it works
- Fill in the form below with the email you used to answer the survey.
- You'll receive a confirmation link by email. Click it within 24 hours to validate your identity.
- The Data Protection Officer (DPO) processes the request within 15 calendar days.
- You receive the answer at the same email provided.
When PulseForm is Controller vs. Processor
Important for you to know who to approach:
- Data you submitted in a customer's survey: the customer is the Controller. You can ask PulseForm (channel above), but we may need to redirect you to the Controller or act on their instruction.
- Data from your PulseForm account (paid or free user): PulseForm is the Controller. We handle it directly.
Complaints to data protection authorities
You can submit complaints directly to the Brazilian National Data Protection Authority: gov.br/anpd. We recommend giving us a chance to resolve first.
